The Bone That Breaks — and the Body That Knows How to Mend It
A fracture is one of the body's most dramatic self-repair challenges — and one of its most remarkable achievements. Within hours of a break, the body initiates a cascade of biological processes that will, over weeks and months, rebuild a structure as strong as the original. Haematoma formation, inflammatory response, soft callus formation, hard callus mineralisation, and finally bone remodelling — each phase precisely orchestrated, each dependent on the one before.
Western orthopaedics manages this process brilliantly for complex fractures requiring surgical fixation. But the majority of fractures — wrist fractures, ankle fractures, rib fractures, stress fractures, and many hip fractures in younger patients — are managed conservatively: cast, splint, or boot, with physiotherapy beginning as the bone heals. The body does the work. Western medicine provides the scaffolding.
TCM offers something the scaffolding cannot: a systematic approach to accelerating and optimising the biological healing process itself — through food that provides the raw materials for bone formation, movement that promotes the Qi and Blood circulation that healing requires, and sleep that creates the hormonal environment in which bone repair proceeds at its fastest rate.

What Western Medicine Understands About Fracture Healing
Fracture healing proceeds through four overlapping phases:
- Phase 1 — Haematoma Formation (Days 1–5): Blood fills the fracture gap, forming a clot that provides the scaffold for subsequent healing. Inflammatory mediators recruit the cells that will drive repair.
- Phase 2 — Soft Callus Formation (Days 5–21): Fibroblasts and chondrocytes produce a soft cartilaginous callus that bridges the fracture gap. The bone is fragile but no longer freely mobile.
- Phase 3 — Hard Callus Mineralisation (Weeks 3–12): Osteoblasts mineralise the soft callus, converting it to woven bone. The fracture becomes progressively stronger.
- Phase 4 — Bone Remodelling (Months 3–24): Osteoclasts and osteoblasts remodel the woven bone into lamellar bone, restoring the original architecture and strength.
The factors that most significantly affect healing speed and quality: nutritional status (calcium, vitamin D, protein, vitamin C, zinc), sleep quality (growth hormone is released almost exclusively during deep NREM sleep and is the primary driver of bone remodelling), smoking (reduces bone blood supply by up to 40%), and age (Kidney Jing depletion in TCM terms).
Research published in Bone found that sleep deprivation reduced bone mineral density and impaired fracture healing in animal models — with effects mediated through growth hormone suppression and increased cortisol, which inhibits osteoblast activity.
What TCM Sees: Three Phases of Fracture Recovery
TCM's classical approach to fracture healing — developed over two thousand years of orthopaedic practice — divides recovery into three phases that map remarkably well onto the Western biological understanding.
Phase 1: Activating Blood Stasis (活血化痀) — Days 1–21
The haematoma that forms at the fracture site is, in TCM terms, Blood stasis — static Blood that must be activated and transformed before healing can proceed. The pain, swelling, and bruising of the early fracture period are expressions of this stasis. The primary TCM intervention in Phase 1 is not rest — it is the active promotion of Blood circulation through the fracture site, using foods, herbs, and gentle movement that move Blood without stressing the healing bone.
Phase 2: Connecting Bone and Sinew (接骨續筋) — Weeks 3–8
As the soft callus forms, the TCM focus shifts to nourishing the Liver (which governs the sinews and tendons surrounding the fracture) and the Kidney (which governs the bones). The classical TCM herbs for this phase — Xu Duan (Dipsacus), Gu Sui Bu (Drynaria), and Du Zhong (Eucommia) — are specifically indicated for "connecting bone and sinew" and have been used in Chinese orthopaedic medicine for over a thousand years.
Phase 3: Nourishing Kidney Jing (補腎精) — Weeks 8+
As the hard callus mineralises and remodelling begins, the TCM focus shifts to the deep constitutional nourishment of Kidney Jing — the essence that determines bone density, healing capacity, and long-term skeletal resilience. This phase is particularly important for older adults, whose Kidney Jing reserves are already reduced, and for patients with osteoporosis or previous fractures.
Food Therapy: The Three-Phase Fracture Healing Kitchen 🥗
Phase 1 Recipe: Hawthorn, Turmeric & Black Vinegar Blood-Moving Tea (山楂薑黃黑醋茶)
Ingredients: 15g dried hawthorn berries (Shan Zha), 5g turmeric powder (Jiang Huang), 2 tablespoons black rice vinegar, 300ml hot water, honey to taste.
Method: Steep hawthorn for 10 minutes, add turmeric, vinegar, and honey. Drink warm, twice daily in the first 3 weeks.
TCM Action: Hawthorn moves Blood stasis and reduces the swelling and bruising of the early fracture period. Turmeric (Jiang Huang) is one of TCM's most powerful Blood-activating herbs — its curcumin has the most extensive evidence base of any natural anti-inflammatory compound, with specific research confirming its ability to accelerate fracture healing by promoting angiogenesis (new blood vessel formation) at the fracture site. Black vinegar activates Blood circulation and increases mineral bioavailability. This tea directly addresses the Phase 1 Blood stasis that must be resolved before healing can proceed.
Phase 2 Recipe: Xu Duan, Gu Sui Bu & Pork Bone Broth (續斷骨碎補豬骨湯)
Ingredients: 15g Xu Duan (Dipsacus root), 15g Gu Sui Bu (Drynaria rhizome), 500g pork bones (including marrow bones), 10g ginger, 1.5 litres water, 2 tablespoons black rice vinegar, salt to taste.
Method: Roast bones at 200°C for 20 minutes. Simmer all ingredients for 4–6 hours. Consume the broth daily from week 3 onward.
TCM Action: Xu Duan ("Restore What Is Broken") is the most specifically indicated TCM herb for fracture healing — its name literally means "to continue what is severed." Modern research confirms its osteogenic properties: Xu Duan extracts significantly stimulate osteoblast proliferation and differentiation in vitro. Gu Sui Bu ("Mend the Shattered Bone") has documented bone-density-supporting and fracture-healing properties. Pork bone broth provides collagen, glycine, and minerals; the vinegar dramatically increases mineral extraction from the bones. This broth is the most powerful Phase 2 food therapy intervention available.
Phase 3 Recipe: Black Sesame, Walnut & He Shou Wu Jing-Rebuilding Congee (黑芝麻核桃何首烏粥)
Ingredients: 30g black sesame seeds (toasted), 20g walnuts, 10g He Shou Wu (Polygonum multiflorum, prepared form only), 100g black rice, rock sugar to taste.
Method: Cook as congee for 50 minutes. Eat warm for breakfast daily from week 8 onward.
TCM Action: Black sesame and walnuts are the two premier Kidney Jing tonics in the TCM food pharmacy, with documented bone-density-supporting properties. He Shou Wu (prepared form) nourishes Liver Blood and Kidney Jing — the two foundational substances that determine long-term bone density and fracture resistance. Black rice nourishes Blood and Yin. This congee is the long-term constitutional support that ensures the remodelling phase produces bone of the highest possible density and resilience. Note: use only the prepared (Zhi) form of He Shou Wu; consult a qualified TCM practitioner for appropriate dosage.
Tai Chi & Qi Gong for Fracture Recovery 🥋
Phase 1 (Days 3–21): Microcosmic Orbit Breathing
Exercise: Qi Circulation to the Fracture Site
Lying flat, breathe slowly and deeply. On each inhale, visualise warm golden Qi gathering in the Dan Tian. On each exhale, direct this Qi through the body to the fracture site — visualising it as warm, healing light filling the break and activating the Blood stasis. 10 minutes, twice daily. This practice can be performed from the first days of recovery without any physical movement, and directly addresses the Phase 1 Blood stasis through the Qi circulation that TCM considers the prerequisite for Blood movement.
Phase 2 (Weeks 3–8): Distal Meridian Activation
Exercise: Activating the Meridians That Pass Through the Fracture
Identify the meridians that pass through the fracture site (for a wrist fracture: Lung, Large Intestine, Pericardium, Triple Warmer; for an ankle fracture: Stomach, Gallbladder, Bladder, Kidney, Spleen, Liver). Activate these meridians at points distal to the fracture — above and below the cast or splint — through gentle acupressure and movement. This promotes Qi and Blood circulation through the fracture site without stressing the healing bone. 15 minutes daily.
Phase 3 (Weeks 8+): Progressive Weight-Bearing Tai Chi
Begin with the standing Zhan Zhuang posture — feet shoulder-width apart, knees slightly bent, weight evenly distributed. Progress to slow weight-shifting exercises, then to the opening movements of Yang-style Tai Chi. The slow, controlled weight-bearing of Tai Chi provides the mechanical stimulation that bone remodelling requires (Wolff's Law: bone remodels in response to the loads placed upon it) while the meditative quality reduces the cortisol that inhibits osteoblast activity. A 2004 meta-analysis in Archives of Physical Medicine and Rehabilitation found that Tai Chi significantly improved bone mineral density and reduced fracture risk in older adults.

Growth Hormone and the Bone-Building Night
Growth hormone — the primary driver of bone remodelling, collagen synthesis, and fracture callus mineralisation — is released almost exclusively during deep NREM sleep, with the largest pulse occurring in the first 90 minutes of sleep. A single night of poor sleep reduces growth hormone secretion by up to 70%. For fracture patients, this means that every night of disrupted sleep is a night of significantly reduced bone healing.
TCM identifies the Hai Shi window (21:00–23:00) as the Triple Warmer meridian peak — the period when the body's energy distribution system prepares for the overnight repair processes. Being asleep before 23:00 ensures the body enters the Zi Shi (23:00–01:00) Gallbladder meridian peak — when the deepest NREM sleep and the largest growth hormone pulse occur — in a state of genuine deep sleep rather than light or transitional sleep.

Fracture patients sleep with casts, splints, or boots that create pressure, heat, and discomfort. The limb in the cast is often elevated, creating an awkward sleep position that makes repositioning difficult and disruptive. Step 3 (15 minutes before sleep): Da Zhui (GV14) acupressure — at the base of the neck, between the seventh cervical and first thoracic vertebrae. The meeting point of all Yang meridians — activates the Du Mai and promotes Yang Qi circulation through the entire skeletal system. 2 minutes. Shen Shu (BL23) — the Kidney back-transporting point — for 2 minutes each side. The most direct acupressure point for nourishing Kidney Jing and supporting bone healing.
Step 4: Position the fractured limb comfortably on a silk-covered pillow. Draw the lightweight silk duvet over the body. Sleep before 23:00 — protecting the growth hormone pulse that is the most powerful bone-healing intervention available.
