Why Do Cold Sweats Happen? Plus, 9 Ways to Treat Them

You wake at 3am. The sheets are damp. Your skin is cool and clammy despite the sweat. Your heart is beating faster than it should be for someone who was just asleep. You are not hot — if anything, you feel slightly cold. And yet you have clearly been sweating.
This is a cold sweat — and it is one of the body's most visceral communications. Unlike the warm sweat of exercise or heat, cold sweats are the body's fear response made visible: a physiological alarm signal that something — real or perceived — has activated the nervous system's emergency protocols.
Understanding what causes cold sweats, why they happen at night, and how to treat them requires looking at both the physiology of the stress response and the conditions that trigger it — including some that are less obvious than others.
The Physiology of Cold Sweats
Cold sweats are produced by the sympathetic nervous system's activation of the eccrine sweat glands — the same glands responsible for thermoregulatory sweating, but activated through a different pathway.
In thermoregulatory sweating, the hypothalamus detects elevated core body temperature and signals the eccrine glands to produce sweat, which evaporates and cools the skin. The sweat is warm because the body is warm.
In stress-induced sweating — the cold sweat — the amygdala detects a threat (real or perceived) and activates the sympathetic nervous system, which signals the eccrine glands through cholinergic nerve fibers to produce sweat regardless of body temperature. Simultaneously, the sympathetic system constricts peripheral blood vessels — redirecting blood from the skin to the large muscles in preparation for fight or flight. This vasoconstriction reduces skin temperature, producing the characteristic combination of sweating and coolness that defines the cold sweat.
Adrenaline (epinephrine) plays a central role. The adrenal medulla releases adrenaline in response to sympathetic activation, and adrenaline directly stimulates eccrine sweat gland activity through beta-adrenergic receptors. This is why cold sweats are so closely associated with acute fear, shock, and the moments of sudden alarm that produce the "cold sweat" of common experience.
What Triggers Cold Sweats?
Anxiety and panic. The most common cause of cold sweats in otherwise healthy people. Acute anxiety episodes and panic attacks produce the full sympathetic activation that generates cold sweats — often accompanied by heart palpitations, shortness of breath, and the sense of impending doom that characterizes panic.
Hypoglycemia. Low blood glucose is a potent trigger for sympathetic activation — the brain, which runs almost exclusively on glucose, treats glucose deficiency as an emergency and activates the stress response to mobilize glucose stores. The cold sweats of hypoglycemia are often accompanied by shakiness, confusion, and intense hunger. They are particularly common in the early morning hours (3–5am) when blood glucose naturally reaches its daily nadir.
Hormonal changes. The hot flashes of menopause are well-known, but the cold sweats of perimenopause and menopause are less discussed. Declining estrogen levels disrupt the hypothalamus's temperature regulation, producing episodes of sweating that can occur without elevated body temperature. Similar hormonal-driven sweating can occur in men with low testosterone.
Infection and fever. The sweating that accompanies fever — particularly the sweating that occurs as a fever breaks — can produce cold sweats as the body rapidly dissipates heat. Night sweats associated with persistent infection (including tuberculosis, HIV, and certain cancers) are a recognized clinical sign that warrants medical evaluation when they occur without obvious cause.
Medications. Several common medications produce cold sweats as a side effect, including antidepressants (particularly SSRIs and SNRIs), antipyretics, and certain blood pressure medications. If cold sweats began after starting a new medication, the medication is a likely contributor.
Cardiovascular events. Cold sweats are a recognized warning sign of myocardial infarction (heart attack) — produced by the massive sympathetic activation that accompanies cardiac ischemia. Cold sweats accompanied by chest pain, jaw pain, left arm pain, or shortness of breath require immediate emergency evaluation.
Why Cold Sweats Disrupt Sleep
Nocturnal cold sweats — cold sweats that occur during sleep — are among the most disruptive sleep disturbances available. Their impact on sleep quality operates through several mechanisms.
Thermal disruption. Sweating during sleep raises skin moisture and, as the sweat evaporates, lowers skin temperature below the optimal range for continued sleep. The resulting thermal discomfort — the clammy sheets, the cooling skin — triggers arousal from sleep, often to full wakefulness.
Sympathetic activation. The sympathetic nervous system activation that produces the cold sweat is itself incompatible with the parasympathetic dominance required for deep sleep. The cold sweat is both a symptom of sympathetic activation and a perpetuator of it — the physical discomfort of waking damp and cold further activates the stress response, making return to sleep difficult.
Sleep architecture disruption. Cold sweats most commonly occur during REM sleep — when the body's thermoregulatory system is partially suspended and the emotional processing of the day's experiences is most active. REM-associated cold sweats fragment the sleep architecture at precisely the stage most important for emotional regulation and memory consolidation.
The TCM Framework: Yin Deficiency and Wei Qi Failure
Traditional Chinese Medicine has a sophisticated framework for understanding sweating disorders that maps onto the Western physiological model with characteristic precision.
The primary TCM pattern associated with cold sweats and night sweats is Yīn Xū (陰虛) — Yin Deficiency. In TCM, yin represents the body's cooling, nourishing, and anchoring resources — the counterbalance to yang's warming and activating energy. When yin is deficient — depleted by chronic stress, overwork, insufficient sleep, or constitutional weakness — it cannot adequately contain the body's yang energy, which rises and generates the heat that drives sweating even in the absence of elevated body temperature.
Night sweats specifically are associated with Yīn Xū Fèi Rè (陰虛發熱) — Yin Deficiency with Deficiency Heat — a pattern in which the insufficient yin allows a pathological heat to arise from within, most active during the yin-dominant nighttime hours when the body's yang should be at rest.
A secondary TCM pattern relevant to cold sweats is Wèi Qì Bù Gù (衛氣不固) — Defensive Qi Insecurity. Wei qi is the body's defensive energy that circulates on the surface, regulating the opening and closing of the pores. When wei qi is deficient or unstable, the pores cannot be properly regulated — opening inappropriately and allowing sweat to escape without the normal thermoregulatory trigger. This pattern is associated with spontaneous sweating (cold sweats during waking) and is worsened by anxiety, which further destabilizes the wei qi's regulatory function.
The cold sweat is the body's honest report: something has alarmed it deeply enough to activate its emergency systems. The work is to find what that something is — and to address it at its source rather than merely drying the sheets.
9 Ways to Treat Cold Sweats
1. Address the underlying anxiety
For cold sweats driven by anxiety, treating the anxiety is the primary intervention. Evidence-based approaches include Cognitive Behavioral Therapy (CBT), mindfulness-based stress reduction (MBSR), regular aerobic exercise, and — where appropriate — pharmacological support. Reducing the baseline sympathetic activation that drives anxiety reduces the frequency and intensity of cold sweats as a downstream consequence.
2. Regulate blood glucose before bed
For cold sweats that occur in the early morning hours (3–5am), hypoglycemia is a common and frequently overlooked cause. A small, protein-containing snack before bed — a handful of nuts, a small amount of cheese, or a hard-boiled egg — stabilizes blood glucose through the night and reduces the sympathetic activation that hypoglycemia triggers. Avoiding alcohol before bed also reduces nocturnal hypoglycemia, as alcohol impairs the liver's glucose-releasing function during sleep.
3. Cool the bedroom
A bedroom temperature of 16–19°C reduces the thermal load on the body during sleep, lowering the baseline from which any additional heat generation — including that of stress-induced sweating — occurs. A cooler bedroom also supports the core body temperature drop that initiates and maintains deep sleep, reducing the likelihood of the arousal events that cold sweats produce.
4. Switch to moisture-wicking silk bedding
The bedding environment is critical for people who experience nocturnal cold sweats. Cotton and synthetic fabrics absorb sweat and retain moisture against the skin, prolonging the thermal discomfort that prevents return to sleep. Mulberry silk — with its natural protein fiber structure — wicks moisture away from the skin surface while maintaining a stable microclimate temperature. The result is a sleep surface that manages the moisture of cold sweats without the clammy, cold sensation that disrupts sleep. Silk's natural temperature-regulating properties also reduce the thermal fluctuations that trigger sweating in the first place, making it both a treatment for and a prevention of nocturnal cold sweats.
5. Nourish yin with TCM dietary approaches
Foods that nourish yin and reduce deficiency heat are the cornerstone of TCM dietary treatment for night sweats: lily bulb (Bǎi Hé, 百合), tremella mushroom (Yù Ēr, 玉耳), black sesame seeds (Hēi Zhī Ma, 黑芝麻), mulberry fruit (Sāng Shèn, 桑椒), and wolfberry (Gǒu Qǐ Zǐ, 枸杞子). These foods are cooling, moistening, and yin-nourishing — directly addressing the deficiency that allows deficiency heat to arise. Avoid spicy foods, alcohol, and excessive caffeine, which generate heat and further deplete yin.
6. Apply acupressure to Yin Xi (HT6)
The point Yīn Xī (HT6, 陰郴) — located on the inner wrist, one finger-width above the wrist crease on the ulnar side — is the primary TCM point for night sweats and is specifically indicated for yin deficiency sweating. Gentle bilateral pressure for 2–3 minutes before bed can support the yin's capacity to contain the body's heat during sleep. The adjacent point Shén Mén (HT7) can be stimulated simultaneously for its heart-calming and anxiety-reducing effects.
7. Practice the cooling breath (Sitali Pranayama)
The yogic cooling breath — inhaling through a curled tongue or through the teeth, then exhaling through the nose — has a direct cooling effect on the body's core temperature and reduces the sympathetic activation that drives cold sweats. Performed for 5–10 minutes before bed, it supports the yin's cooling function and prepares the body for the temperature drop that initiates deep sleep.
8. Consolidate wei qi with consistent sleep timing
In TCM, wei qi is consolidated during sleep and distributed during waking. Irregular sleep timing — varying bedtimes and wake times by more than 60 minutes — disrupts the wei qi's regulatory cycle, impairing its capacity to stabilize the pores and prevent inappropriate sweating. Consistent sleep timing is both a TCM prescription for wei qi consolidation and a circadian science prescription for sleep quality optimization.
9. Seek medical evaluation for persistent night sweats
Cold sweats that occur regularly without an obvious anxiety or lifestyle cause warrant medical evaluation. Persistent night sweats can be a sign of hormonal imbalance, infection, medication side effects, or — rarely — more serious conditions. A thorough medical history, physical examination, and targeted laboratory testing can identify treatable causes and provide the reassurance that anxiety-driven cold sweats often require.
The body that sweats in the night is not failing. It is communicating — with urgency, in the only language it has. Learning to listen to that language, rather than merely managing its symptoms, is the beginning of genuine healing.
